For those individuals that were born in India and/or previously held Indian citizenship and are naturalized citizens of their current country, in addition to applying for your OCI, you will need to complete the steps required for renunciation of your Indian Citizenship (if you have not done so already).
Either of the following are proof of renunciation of Indian citizenship:

1) A certificate of renunciation issued by a Consulate of India; or
2) Your cancelled Indian passport with stamp stating that the Indian Passport is cancelled due to obtaining US (or any other non-Indian country) citizenship / nationality.
FORMS:
If you cannot show proof of renunciation, as described above, or have NOT already renounced your Indian Citizenship, the following must be provided:

1) Copy of your naturalization certificate

Note: If a minor gained naturalization through his/her parents and possesses no separate naturalization certificate, then a written statement signed by both parents is required stating the circumstances that their child received his/her naturalization status and why the child does not possess his/her own certificate. This should be accompanied with a copy of the parent(s) naturalization certificate.

2) If the most recent Indian passport is available, provide the following:

i) Renunciation Certificate Form - U.S. Form [See Sample]

3) If you do not have your most recent Indian passport, you must provide:
(i) Affidavit of Loss - U.S. Form [See Sample]

AND ONE OF THE FOLLOWING:

(ii) If you were naturalized over 10 years from today, you must provide a Deemed Surrender Certificate - U.S. Form [See Sample]

OR
-
(iii) If you were naturalized after June 1, 2010 OR less than 10 years from today, you must provide a Renunciation Declaration - U.S. Form [See Sample]

Once you have gathered and completed the necessary documents, you will need to submit them, along with the appropriate fees, to the appropriate Indian Mission.
